Traduction rapport d’enquête - (Aircraft 228 was the lead plane in a flight of 4 aircraft. Aircraft 044 was the fly number four position, slightly below and behind number 228. Conditions necessitated instrument ascent and forming on top of the clouds (above 3000 ft). It was during this join-up procedure that this accident occured. 228 as flight lead was to fly at 185 mph on a east-west course (from a westerly heading to an easterly heading) when the accident occured. As 044 could not locate the flight leader he followed 903 who was to be in the same fomation. Both 903 and 044 were overtaking 228... the pilot of 044 apparently did not see 228 but see 903 lower his wheels (in an attemps to slow his aircraft up and join formation) and this might have attracted his attention, as it is an unusual occurence. 228 was in a turn to the left, as was 044, when the impact occured. Speed of 044 was greater than that of 228 and the right prop apparently knocked off the vertical stabilizer of 228 and continued chewing into the airplane. It is probable that the pilot of 044 had never seen 228 below him. At least the fact that his airplane was banked to the left would indicate that it is improbable he could see 228 at the time of impact. Responsability for this accident is placed 100% pilot error on the part of Lt Clevenger in aircraft #044.Original) - (source : AFHRA – Accident Report) L'avion 228 était l'avion de tête d'un vol de 4 appareils.
