Traduction du rapport d’enquête – 1Lt Caldwell - (The strip was not completed, only half of the width having been paved with wire matting. The squadron had been ordered to operate out of the strip by higher headquarters. The squadron was thoroughly briefed as to the hazards of taking-off under the existing conditions which were further complications by extreme dust conditions. [...] Lieutenant Caldwell lined up with the strip and started his roll for take-off with full power. Just before he obtained speed he felt a severe jolt on his right wheel. The plane was thrown into the air. Lieutenant Caldwell attempted to keep the plane in the air, however he did not have sufficient flying speed and the plane came back to the ground swerving to the right where the Engineers were working. The plane hit two of the Engineers killing one and severely injuring the other.[...]Original) - (source : Accident Report – AFHRA): La piste n'était pas terminée, seule la moitié de la largeur avait été pavée de treillis métallique. L'escadron avait reçu l'ordre d'opérer hors de la bande par le quartier général supérieur. L'escadron a été parfaitement informé des dangers du décollage dans les conditions existantes, qui étaient encore plus compliquées par des conditions de poussière extrêmes. […] Le lieutenant Caldwell s'aligna avec la piste et commença son roulement pour le décollage à pleine puissance. Juste avant d'atteindre la vitesse, il a ressenti une forte secousse sur sa roue droite. L'avion a été projeté en l'air. Le lieutenant Caldwell a tenté de maintenir l'avion en l'air, mais il n'avait pas une vitesse de vol suffisante et l'avion est revenu au sol en faisant une embardée vers la droite où travaillaient les ingénieurs.
